Integrated Care Nurse – FSP-ICM

Integrated Care Nurse for the Full-Service Partnership (FSP-ICM) program at BRIDGES, INC. in Woodland Hills, CA. The FSP is an intensive, wraparound program for adults with severe mental illness, often with co-occurring substance use. The role involves delivering integrated services in the community including clients' homes, shelters, streets, and hospitals. Schedule is 20-25 hours/week (field/office). Bilingual Spanish-English preferred.
