About this role
Advanced Care Partners is seeking a Registered Nurse (RN) to provide individualized care for medically fragile children and adults in home and school settings. This position offers a more personalized care model, allowing for stronger patient outcomes and professional satisfaction. Key responsibilities include providing skilled nursing care (feeding tubes, tracheostomies, ventilators, central lines), monitoring patient progress and responding to changes in condition, educating patients and families on treatment plans, and maintaining a safe and clean care environment.

Requirements
- Active RN license in Georgia or Nurse Licensure Compact (NLC)
- Minimum of one year of RN experience
- Current Basic Life Support (BLS) certification through American Heart Association (AHA) or equivalent accredited organization
- Valid PPD skin test
- Ability to stand or sit for extended periods (up to 12 hours)
