About this role
RN Case Manager position conducting all aspects of supervision and management of patient care in home health, including LVN and HHA supervision, case conferences, and discharge planning. Promotes patients' potential during their home health episode and collaborates with physician, clinical team, and patient in preparation for discharge. Supervises and manages all aspects of patient care, coordinates distribution of visits among all disciplines, leads weekly interdisciplinary case conferences, and coordinates discharge planning.
