RN Case Manager

Case manager is responsible for providing, coordinating, monitoring, and evaluating treatment plans and their progress; facilitating interdisciplinary approaches; and monitoring progress for patients covered under worker's compensation. Work closely with physicians, nurses, mental health and other health care professionals, as well as family of patients. Maintains client's electronic file. Makes appropriate referrals for patients to obtain needed services. Creates various reports. Extensive documentation is necessary to ensure that the individual receives needed services and makes progress towards goals. This position includes telephonic and field case management. This position includes report writing and summarizing documents.
