Full-Time Registered Nurse (RN) – Home Care Assessments

*Anchor Health Homecare Services* *Overview* We are hiring a Full-Time RN to perform in-home patient assessments across our service area. This role offers a steady caseload with the independence of field work and the support of an experienced clinical team. If you enjoy autonomy but still want stability, this is a strong fit. *What You’ll Do* * Conduct in-home nursing assessments (initial evaluations, reassessments, and PRN visits) * Evaluate patient condition including physical, cognitive, and psychosocial status * Review medications and assess safety risks and fall risk * Complete documentation within: * 48 hours (initial & PRN visits) * 5 days (reassessments) * Collaborate with physicians and care team to ensure continuity of care *What You Need* * Active RN license (in-state) * Minimum 1 year of nursing experience * Home health or case management experience preferred * Reliable transportation and valid driver’s license * Strong time management and documentation skills *What We Offer* * Competitive compensation * Consistent, full-time caseload * Weekly direct deposit
