About this role
Optum Home & Community Care Field Case Manager RN position. Visit members in homes to assess health status, identify gaps or barriers in treatment plans, provide patient education, make referrals, and support members with condition education and medication reviews. Field based position throughout Seattle, WA. Monday through Friday 8:00 a.m. - 5:00 p.m. PST. $7,500 Sign On Bonus for External Candidates.

Requirements
- Current unrestricted RN license in Washington, BLS Certification or ability to obtain by start date, 1+ years clinical experience, Experience in home care/home visits, Reside in or near Seattle WA, Driver's license and access to reliable transportation, BSN, Certified Case Manager (CCM), Case management experience, Experience in discharge planning, Experience in utilization review concurrent review or risk management, Experience in telephonic role, MS Word Excel and Outlook, Background in managed care, Bilingual English/Spanish
