Registered Nurse - RN - Home Health - PRN

Administer nursing care for clients in their residence, coordinate care with the interdisciplinary team, clients and their families, and the referring agency. Complete initial and ongoing assessments to identify physical, psychosocial, and environmental needs of home health clients. Regularly reevaluate client nursing needs and evaluate outcomes of care. Develop, initiate, and revise the Plan of Care. Provide effective communication to clients, families, team members, and other healthcare professionals. Apply infection control and universal precaution. Coordinate services and inform physician of changes. Complete appropriate documentation. Supervise, teach, and provide clinical direction to other nursing personnel. Perform on-call responsibilities.
