About this role

Home Health Registered Nurse (RN) providing home-based care to patients with illnesses, injuries and chronic diseases after discharge from hospital, rehab or nursing facility. Work with physicians and oversee LPNs, CNAs and aides. Examine patients, do assessments, document progress, educate patients and family members on care plans, and coordinate discharge from services.

Graduate of an accredited nursing program
Active RN license in AZ
Minimum of 1 year of nursing experience, ideally in home healthcare
