Registered Nurse

The RN reports to the Clinical Manager and is accountable for providing care management for high-risk populations with an emphasis on event-driven care leveraging care pathways and evidence-based guidelines. Responsibilities include conducting in-home and telehealth assessments, implementing nursing care plans, managing patient care transitions, coordinating care for patients with complex chronic medical and mental health conditions, deploying remote patient monitoring, and collaborating with a multi-disciplinary clinical team to deliver personalized care in home-based and telehealth settings. Qualifications include BSN required (MSN preferred), 3-5 years clinical experience, unrestricted RN licensure in state of hiring location, preferably eNLC compact-multi-state license.
