Hospice Registered Nurse (RN)

You take pride in creating personalized care plans for patients and are able to observe and communicate effectively in your patient's best interest. To succeed, you should have demonstrated knowledge and skill in current nursing practices as well as a strong penchant for solving problems. Responsibilities Work as part of a team on hospice and palliative care teams who provide comfort and support to patients and their families during a terminal episode of illness. Work with nursing staff to coordinate patient care and discharge planning. Be skilled in the management of pain and symptom management. Knowledge in diagnosing conditions, medication management and communication between care providers. Be able to explain medications to patients and families. Responsible for all aspects of coordination including, but not limited to, scheduling appropriate follow-up appointments with health care providers, managing medications, referrals and hospital admission. Provide assistance in identifying, qualifying and documenting resources. Qualifications Currently licensed Registered Nurse (RN) in Texas Previous experience as a Registered Nurse is preferred Excellent interpersonal and client care skills Valid driver’s license and Automobile Insurance First aid/CPR certification
