About this role
The Nurse Assessor is responsible for conducting accurate assessments for consumers seeking community-based long-term care services and support. Responsibilities include performing initial assessments of consumers that includes evaluation of health status, functional status, strengths, care needs, and preferences, and reviewing consumer medical documentation and/or health records.
