About this role
*Job Summary*
We are seeking a dedicated and skilled Family Nurse Practitioner (FNP) to join our healthcare team. The ideal candidate will provide comprehensive patient care, focusing on health promotion, disease prevention, and management of acute and chronic illnesses. As an FNP, you will play a critical role in delivering high-quality healthcare services to patients of ages 2 and up, ensuring their well-being and fostering a positive patient experience.

Monday, Wednesday, Friday: 7:30 AM – 4:00 PM
Tuesday & Thursday: 10:30 AM – 7:00 PM
*Responsibilities*
* Conduct thorough physical examinations and health assessments for patients.
* Diagnose and treat a variety of medical conditions across all age groups.
* Develop and implement individualized care plans based on patient needs.
* Collaborate with interdisciplinary teams to ensure comprehensive patient care.
* Educate patients and families on health maintenance, disease prevention, and treatment options.

* Master’s degree in nursing from an accredited program.

* Current certification as a Family Nurse Practitioner by a recognized certifying body.
* Valid state licensure to practice as a Nurse Practitioner.
* DOT certification or willingness to obtain.
* Strong knowledge of patient care protocols and best practices in nursing.
* Excellent communication skills with the ability to build rapport with patients, families and co-workers.
* Ability to work collaboratively within a healthcare team to provide optimal patient outcomes.
* Experience with electronic health records (EHR), specifically eCW, is preferred but not mandatory.
