About this role

The Hospice Registered Nurse Case Manager delivers professional nursing care to the agency's home care patients as directed by the physician and in accordance with the State Nurse Practice Act and agency policies and procedures. Additionally, the RN may provide nursing services in an institutional setting while adhering to that institution's policies and procedures.

Requirements

Current Registered Nurse (RN) license in the State of Oklahoma
Minimum of two years of hospice nursing experience preferred
Certification in Hospice and Palliative Nursing (CHPN) highly desirable
Strong verbal, written, and electronic communication skills
Valid driver's license
Reliable transportation
Active auto liability
Ability to lift 50 pounds
