Responsibilities include working collaboratively with physicians, nurse practitioners, nurses, therapists, social workers, and caregivers to deliver comprehensive and coordinated care to patients in their homes, conducting patient home visits, assessing and planning patient care plans, managing emergency situations, completing medication administration and treatment needs, providing nursing care in accordance with regulations and company policy, and maintaining accurate clinical and progress notes.
