Director of Nursing (Home Health)

VitalTrust Home Health LLC is seeking an experienced, compassionate, and organized Director of Nursing (DON) to assist with our upcoming ACHC accreditation survey and continue in a part-time leadership role afterward. This is an excellent opportunity for an experienced home health nurse who enjoys building programs, ensuring regulatory compliance, and helping establish a high-quality home health agency. Responsibilities include serving as the Director of Nursing for a Medicare-certified home health agency, assisting with ACHC accreditation preparation and survey readiness, developing and maintaining clinical policies and procedures, ensuring compliance with ACHC, CMS, and state regulations, providing clinical oversight for skilled nursing services, reviewing patient documentation, participating in QAPI, supervising clinical staff, and collaborating with the agency administrator.
