About this role
Provides planning and delivery of direct and indirect patient care through the nursing process of Assessment, Planning, Intervention, and Evaluation. Develops nursing care plans in coordination with patient, family and interdisciplinary staff. Communicates changes in patient's clinical condition with Physicians, Nursing Supervisor/Manager, and co-workers. Participates in discharge planning process. Maintains standards of nursing care, directs and supervises nursing care provided to patients, administers medications, assists physicians with examinations and procedures, and performs patient care responsibilities in a long-term acute care hospital setting.
