OVERVIEW:
Guides patients through the health care system by assisting with access issues, developing relationships with service providers and tracking interventions and outcome. Promotes a positive teamwork environment which emphasizes respect to all customers, including patients, families, physicians, staff, and other departments. Receives and responds to calls from patients and families for health care assistance using the nursing process in accordance with applicable scope and standards of practice and with the policies, values, and mission of the organization. This position will also work collaboratively with clinical care teams in the coordination of care for certain patient symptoms and conditions.

WORK HOURS: Monday - Friday; hours vary by posting, hybrid

Graduate of an accredited nursing program. Bachelor of Nursing degree, preferred.
