Cornerstone Specialty Hospitals Conroe is a long-term acute care (LTAC) hospital specializing in the treatment of patients recovering from post-intensive care and medically complex conditions. Provides planning and delivery of direct and indirect patient care through the nursing process of Assessment, Planning, Intervention, and Evaluation. Develops nursing care plans in coordination with patient, family and interdisciplinary staff. Communicates changes in patient's clinical condition with Physicians, Nursing Supervisor/Manager, and co-workers. Participates in discharge planning process.
