About this role
Hospice RN Clinical Manager/Team Director at Moments Hospice. Lead and oversee an interdisciplinary team of nurses, aides, social workers, chaplains, and more. Ensure adherence to hospice regulations and standards, facilitate communication between medical staff and the care team, lead interdisciplinary meetings, develop patient care policies, manage staff scheduling and performance reviews, and act as central hub for communication with families, physicians, and other providers.

Requirements
- Registered Nurse (RN) license with a minimum of 3 years of healthcare experience, preferably in hospice or related settings Bachelor's degree strongly preferred Leadership or management experience a major plus Valid driver's license (required)
