CCT/CCA Registered Nurse

Access TLC is currently seeking a Registered Nurse (RN) to join our team in the CCT/CCA department. The Registered Nurse (RN) is responsible for assessing, and developing a person-centered care plan and evaluating each case and the services needed according to requirements of the California Community Transitions (CCT) program. This position involves a combination of in-person visits and remote work. The program is designed to assist Medi-Cal beneficiaries to remain in their community as an alternative to a skilled nursing facility and provides specified benefits to eligible seniors and persons with disabilities. The goal of the program is to facilitate a safe transition of Medi-Cal eligible persons from a nursing facility to a community home-like setting. This position is per diem and visit rate is $140 per visit depending on the type of visit. Mileage is also reimbursed.
