In this rewarding role, you will assess the health-related needs of our clients by monitoring ongoing health as well as provide health services for clients. • Follow up on any health-related referrals made • Maintain a tracking system to monitor patient health issues, medical appointments, and prescribed medications (internal/external) • Provide patient and staff education around health-related issues. • Provide crisis intervention on an emergency basis when indicated. • Complete documentation and reports in a timely manner. • Participate in staff meetings, case conferences and other unit meetings. • Maintain communication and teamwork with clinical staff to integrate and enhance clinical services. We require: • Valid NYS RN Licensure • Experience working as a Registered Nurse in a substance use disorder treatment facility setting preferred • Compassionate, organized leader with excellent communication skills Outreach: Where Passion Meets Impact.
