About this role

The registered nurse plans, organizes and directs home care services and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities. Responsibilities include completing initial assessments, developing care plans, administering medications and treatments, counseling patients and families, acting as Case Manager, and supervising home health aides. Qualifications: Graduate of an accredited school of nursing, 1-2 years recent acute care experience, current Colorado licensure and CPR certification, Bachelor's degree with 1 year home health care experience preferred. Must have valid driver's license and insured automobile. Job Types: Full-Time. Medical specialties: Home Health. Work setting: In-person.
