HOME HEALTH NURSE

The home health registered nurse is responsible for providing skilled nursing care to patients in their homes, following a physician-approved plan of care. The nurse assesses patient needs, develops and implements care plans, educates patients and caregivers, and coordinates care with an interdisciplinary team to promote patient independence and optimal health outcomes. Key Responsibilities: Patient Assessment & Care: Conduct initial and ongoing comprehensive assessments of patients in their home environment. Develop, implement and evaluate individualized plans of care. Perform skilled nursing procedures, including wound care, urinary catheters, medication management, injections, and ostomy care. Documentation & Compliance: Maintain accurate, timely and complete electronic medical records (EMR) in compliance with agency policies, state regulations, and Medicare guidelines (including OASIS documentation).
