About this role

Perpetual Care Home Health is seeking for per diem RN Case Manager to join our team in delivering excellent care and services to our patients. The RN Case Manager will cover areas in Alameda and Contra Costa counties such as Pleasanton, Dublin, Livermore, San Ramon, Danville, Alamo, Walnut Creek and other surrounding areas. Compensation: PER VISIT RATES depending on the types of visits. The Registered Nurse plans, organizes and directs home care services and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.

Requirements

Current and Active Professional Licensure to Practice in the State of California

Current valid driver's license

Maintains a current CPR certification

Self-directed and able to work with minimal supervision

Demonstrates excellent observation, problem solving, verbal and written communication skills

Shows ability to organize and prioritize workload independently

Bilingual or Multilingual is an asset

At least 1 year clinical or home health experience required
