Responsibilities: * Provide direct patient care, including assessments, interventions, and evaluations, in alignment with facility policies and clinical standards * Develop and implement individualized care plans in collaboration with physicians, therapists, and interdisciplinary team members * Administer medications and treatments as prescribed * Monitor and report changes in patient condition while maintaining accurate medical records * Partner with team members to ensure comprehensive, holistic care
