RN / Registered Nurse - Harborview's Pediatrics Clinic

Harborview Medical Center (HMC)'s Pediatrics Clinic has an outstanding opportunity as an RN Care Manager. 100% FTE, 40 hours per week, Day shift. Harborview's Pediatrics Clinic provides primary care for infants, children and teens. The clinic utilizes a multidisciplinary staff that is highly collaborative and supportive, working together to achieve the best patient outcomes possible. Responsibilities include collaborating with the interdisciplinary team to identify, engage, assess, plan, implement, coordinate, monitor, and evaluate options and services in meeting the needs of newly arrived pediatric patients. Work closely with the clinic, community agencies, schools, hospitals, and function across the continuum of care. Coordinate and manage the care of newly arrived medically complex children for up to one year including close partnership with resettlement agencies and Seattle Children's. Role includes coordination of DME, referrals to SCH and other community medical organizations. Collaborate with schools and community based services.
