About this role
The Home Health Registered Nurse (RN) provides skilled nursing care to patients in their homes, assessing patient needs, planning and implementing care, coordinating with the care team, and educating patients and their families. The RN ensures quality care while meeting federal, state, and agency standards. Essential functions include completing an initial nursing evaluation within 48 hours of referral per Kansas regulations, conducting ongoing assessments of patient condition during home visits, developing and updating individualized plan of care in collaboration with physicians and interdisciplinary team, performing skilled nursing duties including wound care, IV therapy, catheter care, medication administration, and chronic disease management.
