Oncology Clinical Nurse - TGH Cancer Center Hematology Oncology

Collaborates closely with physicians, advanced practice providers, navigators, pharmacists, and other interdisciplinary team members to ensure timely and coordinated care. Educates patients on pre-treatment requirements, post-procedure expectations, and survivorship or ongoing care needs as appropriate. Works collaboratively with Oncology Nurse Navigators and support services to ensure seamless transitions across settings and phases of care. Facilitates timely access to care and reduces delays by proactively managing follow-up, referrals, and coordination activities. Responds to patient and caregiver inquiries through phone or electronic communication, providing clinical guidance, education, and escalation per protocol. Accurately documents assessments, interventions, patient education, and care coordination activities in the electronic medical record. Practices within the scope of Registered Nurse licensure, adhering to evidence-based oncology nursing standards. Engages in ongoing professional development to maintain oncology knowledge, clinical competency, and certification readiness.
