Primary RN - Case Manager-Hospice

* Monitor patient progress and adjust care plans as necessary * Facilitate discharge planning and ensure appropriate follow-up care * Educate patients and families on treatment plans, medications, and healthcare needs * Maintain accurate and timely documentation in accordance with facility policies * Participate in on-call rotation and provide weekend back-up coverage when required * Requirements: * * Active *Registered Nurse (RN) license – Colorado (or compact state)* * Minimum *1–2 years of RN Case Management or relevant clinical experience preferred* * Strong knowledge of care coordination and discharge planning * Excellent communication and organizational skills * Ability to work independently and as part of a multidisciplinary team * BLS certification (preferred/required as per facility guidelines) *Work Environment:* * Small facility setting (0–49 beds) offering a collaborative and patient-focused environment * Opportunity to work closely with a dedicated healthcare team *Additional Information:* * Monday–Friday schedule offers work-life balance * Occasional on-call and weekend support required * Ideal for RNs seeking case management experience in a smaller healthcare setting Pay: $58.00 - $60.00 per hour Expected hours: 40.0 per week Benefits: * Wellness program Work Location: On the road
