About this role
Chronic Care Coordinator, RN at AllCare Health with the Population Health department. This position is responsible for delivering evidence-based wellness promotion and self-care management solutions to all AllCare Medicaid, Medicare and Medicare dual eligible members of all ages who have chronic diseases and/or behavioral health conditions. Duties include assessment, planning, implementing, coordinating and requesting services to meet member's health care needs, acting as an advocate and facilitating collaboration with the interdisciplinary care team, conducting thorough planning to determine and document specific objectives, goals, and actions to meet member's identified needs including re-evaluation of the individualized care plan.

Requirements
- RN License, Associate Degree in Nursing, BLS Certification, CPI Certification, First Aid Certification, 1 year experience, Driver's License, Spanish, Managed care organization experience, Experience working with trauma survivors, Experience working with individuals with chronic illnesses, Experience working with individuals with mental health conditions, Managing patients as a nurse case manager, Care planning in social services, Initial patient assessment, Clinical assessment documentation, Nursing care plan documentation
