MAT Registered Nurse/Care Manager

Sun River Health is seeking a full-time Registered Nurse/MAT Care Manager to join its team. The RN/MAT Care Manager is part of a multidisciplinary team that uses the nursing process to assess, plan, implement, and evaluate patient care and resource use. Responsibilities include monitoring quality of care, serving as patient advocate, providing care management and crisis management, understanding harm reduction approaches, completing MAT intakes and care plans, triage, opioid overdose prevention education, case management, and coordination with external agencies.
