* Initiating appropriate preventative and rehab nursing procedures, * Preparing and submitting timely clinical and progress notes, * Overseeing coordination of patient services, * Completing medical history/home safety check/environmental assessments, * Performing socio-psychological evaluations, * Developing an individualized Plan of Care (POC) to be submitted to the physician for approval, * Determining medical necessity for other services, * Evaluating the patient’s ADL and IADL abilities and need for a home health aide, * Supervises HHA services and associated requirements and documentation, * Review billing processes, including co-pay requirements, and * Exceptional communications with clients, co-workers, and the general public. * Quality Assurance Experience (QA) * Qualifications: * * Graduate of an approved professional nursing program (RN) (Required) * Licensed “in good standing” in North Dakota & Minnesota (Preferred) * Experience in the home healthcare industry (Required) * Organized and meticulous with an exceptional level of confidentiality and attention to detail * Clean criminal background check and drug screening * CPR certification * Benefits: * Nizhoni offers benefits including insurance (after 30 days of employment) to all full-time employees, and generous time off to ensure that our employees have a healthy work/life balance.
