Position Overview We are seeking experienced and compassionate Registered Nurses (RNs) to perform in-home visits across all NYC boroughs. As a Fee-for-Service Registered Nurse, you will play a vital role in patient care, assessments, and care coordination—helping clients live safely and comfortably in their homes. Compensation (Per Diem Rates) Initial Assessment Visit: $130 Reassessment Visit: $100 Resumption of Care Visit: $100 Supervisory Visit: $80 Key Responsibilities Conduct initial and follow-up in-home nursing assessments Develop and update individualized care plans Educate patients and families on disease management and self-care Supervise and instruct caregivers and home health aides as needed
