As an RN, you'll assess, plan, and manage patient care while collaborating with a multidisciplinary team that shares your commitment to quality. Key Responsibilities: * Conduct comprehensive initial and ongoing assessments based on individual patient needs and provider orders * Develop and continually update care plans to reflect evolving conditions and goals

* Deliver skilled nursing interventions within the scope of practice and in accordance with physician directives * Maintain detailed and compliant documentation per all state, federal, and agency regulations * Lead case conferences, promoting collaboration between caregivers and clinicians * Provide culturally sensitive education to patients and families regarding condition management and treatment plans * Advocate for patient dignity, preferences, and well-being at every step of the care journey What You'll Need: * Graduate of an accredited Nursing Program (RN) * Active Registered Nurse license in MA * Valid CPR/BLS certification * Reliable transportation & valid driver's license * 1+ year of experience in acute care, home health, outpatient, or community health (within the last 2 years) * Up-to-date health certificate & TB test * Clear understanding of HIPAA and strong commitment to confidentiality * Fluent in English (spoken & written) * Must be at least 18 years old and eligible to work in the U. S.
