About this role
The Hospice Admission Nurse is responsible for evaluating patients for hospice eligibility, completing comprehensive admission assessments, coordinating the admission process, and providing education and support to patients and families as they transition to hospice care. This position requires strong clinical judgment, compassionate communication, excellent organizational skills, and the ability to independently manage hospice admissions from referral through completion. Schedule Options: Option 1: Four 8-Hour Shifts (starts at 10:00 a.m., one weekend day required). Option 2: Three 10-Hour Shifts (9:00 a.m. to 8:00 p.m., one weekend day required).

The Hospice Admission Nurse will review hospice referrals, evaluate patients for hospice eligibility, complete comprehensive admission assessments, obtain and document medical history, educate patients and families, complete required admission documents, collaborate with physicians and hospice providers, coordinate medications/equipment/supplies, develop initial plan of care, provide skilled nursing interventions, ensure documentation compliance, provide handoff communication, participate in staff meetings, maintain patient confidentiality, and travel throughout the service area.

Requirements
Current and unrestricted Registered Nurse license in the State of California
Minimum of one year of professional nursing experience
CPR Certification
BLS Certification
CHPN
Driver's License
Reliable transportation
Strong clinical assessment, critical-thinking, and care-coordination skills
