RN Case Manager - Full Time

Helps educate and coach care-giving staff and assists with new hire clinical training, as needed. RN Case Manager Requirements: Current unrestricted RN license in Minnesota Minimum two (2) years of clinical experience with one (1) year of experience in a home care agency, primary care clinic, or health facility, preferred. Critical Care Experience (1-2 years) preferred Active CPR Certification Negative TB skin test or QuantiFERON-TB Gold (QFT) blood test (completed within the last 90 days) Reliable car & Auto Insurance Valid Driver's License Clean background and Criminal Record Understands regulatory compliance with state and federal regulations.
