Clinical Team Manager provides oversight of all patient care services and personnel including making patient and personnel assignments, coordinating patient care, coordinating referrals, assuring patient needs are continually assessed, and assuring the development, implementation, and updates to the individualized patient plan of care.

Requirements
Associate degree in a Nursing related field required
3-5 years of Skilled Home Health Experience including field visits and case management
Experience with Oasis documentation and assessments
1 year of supervisory experience preferred
Current, unrestricted RN license valid for the state of work
Proficiency in enterprise applications such as Homecare Homebase, Workday, and Microsoft Office
Able and willing to travel within branch/office coverage area
Dependable vehicle, valid driver's license, and current auto insurance
