Hospice Quality RN

* Responsible for Quality measures identification and reporting. * Responsible for Quality Assurance and Performance Improvement (QAPI) reports and preparation for monthly Quality Outcomes Committee meetings * Incorporates information learned from tracer and survey activities into Performance Improvement programs * Utilizes system-wide and facility-wide dashboards to track Quality & Safety activities * Liaison to clinical departments identifying and assessing problems and monitoring actions. *Requirements* * Valid nursing license (RN) in good standing with state licensing board. * Proven experience in hospice & palliative medicine or related fields such as critical care, ICU, emergency medicine, or medical-surgical units. * Minimum of five (5) years’ experience in Nursing and minimum of two (2) years of experience in Quality Assessment, Patient Safety or Performance Improvement, required. * Knowledge of and experience with data, analysis and reporting. * Experience with EMR/EHR systems. * Excellent communication skills for triage calls, family education sessions, case management coordination, and interdisciplinary collaboration. * Knowledge of infection control practices and aseptic techniques essential for safe patient care delivery. * Ability to work independently in home settings while maintaining high standards of safety and professionalism.
